This may be a bug, but I don't know, so don't bite my head off.

On stable islands the dukes of cities have that title appear twice in messages, contact lists, etc. So there'll be "Sir Maurus Barek, Duke of Nocaneb, Duke of Nocaneb, Ambassador of Arcaea". Is this a bug or something to do with the new estate system, where the lord of a city and the duke of a duchy are two separate titles?

Indirik

Yes, it is a known bug. It's a relic of the way the messaging system works, and some of the code that is shared between testing and stable islands. In order to get the separation of Duke and Lord-of-a-city titles to work properly on Testing islands, some common code had to be changed. This resulted in the "Duke of Kepler, Duke of Kepler" problem on stable islands. It will get fixed when the new estate system is ported to stable islands.
If at first you don't succeed, don't take up skydiving.

Hasn't that code been on testing for ages, though? This Duke issue on stable has only appeared in the last couple of days.

The signatures were only changed two days ago. The system has been like this for longer, but did not show up in signatures, leading to confusion.

After all it's a roleplaying game.

Ah, okay. Fair enough. :)
