I've recently noticed this feature. Can this cause injuries? or improve your sword-fighting skill? or is it just a fun option that does nothing?

Slight chance to injure, and will not improve but will prevent decay on sword skill.

How many hours do I need to stop the skill decay with this feature?

Not the faintest idea.  And it only works to clear (or partially clear, not sure) the decay from one point to the next down that you already have.  Anything of the "keeps your skills sharp" text does the same thing.

Works more or less just like a normal duel, minus the serious honor business part. It's more or less the duel for sport feature people have wanted so much.

The old Duel till First Blood remade so it can actually be used for sparring :)

Really? I want to duel someone in Astrum but I do not think anyone is willing to let their characters get hurt. :( It is tough to train over 75%.

Training matches do not give you any skill bonuses, and you need to be in a region with an academy. I'd expect the chances of getting injured in such a "duel" is fairly low too, so it's probably safe to go ahead and train, as the name implies.
