Creating a new duchy

I'm sure this must have been asked before, but I can't find it. If a ruler creates a new duchy, is the lord of the "Initial Region" automatically made its duke, or can the ruler choose it?

The Lord of the Region automatically becomes the Duke. It's explained at the Create a New Duchy part.

So it is. I'll blame tiredness for that one. So the only way to have a different duke is for the original one to step down or lose the position somehow?

Correct. You cannot create a new "empty" duchy. The lord chosen is automatically promoted to duke, and his region automatically joins the new duchy, and is removed from the old duchy.
If at first you don't succeed, don't take up skydiving.

Basically "creating a duchy" is really promoting a lord to duke.
