One of my char recently became a duke of a stronghold but I didn't get any fame for it. Does it only apply to a duke of a city?

Fame doesn't apply immediately, if you can wait for the next full turn (might even be next cleanup script, which is a couple of hours after full turn) then check again to see.

Some fame points don't appear right away. Some are delayed longer than others. Guess which fame point has the second longest delay? :P

The prophet fame for 'successful' religions.  The first being one of the Ruler for X days fame points (which I might be missing sadly since all my long lived rulers were a long time ago *sigh* Stuck at #2 for now)
