I think I put that in during my tenure as Ethiala's ruler, or else I induced somebody else to. I don't think I made up the idea though (since we don't have any such rule for Cathay, even though we still label ourselves an 'RP' realm). too lazy to check the wiki to see if I actually added the edit.

I vaguely recall a conversation on the OOC ban among the Titans back in '06 or '07 in which it was mentioned that someone who had an inappropriate name (like, an obviously bad name, not just a 'not great RP name') could/should be OOC banned if they refused to do anything about it.

The in-game OOC ban page itself still says this:

'On the Far-East Island, you are allowed to ban people for repeatedly and consistently refusing to roleplay.'

Ha.

As for the SMA reference, if I did indeed have anything to do with that it would be SMA as it seemed at the time, not SMA as it actually turned out, so that reference is pretty meaningless. In Cathay I try to make a 'serious medieval atmosphere' (lowercase) happen but I don't ever reference SMA.
