The bug hit now on medium risk option too.
Seriously wounded now and lost my unique item.

I've confirmed the bug since I was able to reproduce it. A fatal error occured in the code, that's why it gave you no text and hours weren't subtracted.
It seems like you have hit a case where you were indeed spotted and wounded/serious wounded. But the eventual results were not reported (missing text & no hours subtraction).
We're looking into it.
